Throughout each part, important examples will be given from the literature, followed by a summary of important economic analyses from our field in the last decade. The major limitation of exercise testing, as well as the other techniques, is their rather low sensitivity. Block in the bypass tract during exercise is uncommon, occurring in only <10% of all our patients and <25% of our asymptomatic adults. This likely reflects patient selection bias, but nonetheless, because the vast majority of our patients never develop ventricular fibrillation, I believe that its true sensitivity is low for predicting patients with low risk. Nonetheless, it has a high predictive accuracy of benign prognosis if it is observed. Determination of the Antegrade Refractory Period of the Bypass Tract by Programmed Stimulation One of the major determinants of the rate of conduction over the bypass tract during atrial fibrillation or flutter is 114 the antegrade refractory period of that bypass tract. Wellens and Durrer showed many years ago that the refractory period of the bypass tract exhibited a strong relationship with the shortest R-R or mean R-R interval during atrial fibrillation. Other investigators have shown a less exact but statistically significant correlation. As noted earlier, patients who develop ventricular fibrillation have shorter effective refractory periods than those who 44 do not develop ventricular fibrillation. Although values such as 250 to 270 msec have been used to separate short from long refractory periods, this is artificial, owing to the extreme overlap. Clearly, however, patients whose refractory periods exceed 300 msec, and whose cycle length at which maximum 1:1 conduction over the bypass tract occurs exceeds 300 msec, have an extremely low likelihood of developing ventricular fibrillation. Nonetheless, no single patient in our series who had an antegrade refractory period >300 msec and who failed to conduct 1:1 at cycle lengths <300 msec developed ventricular fibrillation. In our experience, the mean ventricular response during atrial fibrillation correlates best with the maximum rate of 1:1 conduction over the bypass tract, while the shortest R-R interval correlates somewhat better with the effective refractory period of the bypass tract. One must, however, perform refractory period studies at multiple cycle lengths, because the effective refractory period of the 32 125 bypass tract shortens with decreasing cycle lengths.
